Got Sport - requires all paperwork to be completed and APPROVED in order to run a practice, sit on the sidelines at a game - EPYSA has access to this system and if they see a clearance not approved they could forfeit any games in the system for non-compliance.

• Signed and counter signed affidavit (see attached) - this form MUST be used and printed/signed and dated by the manager and their witness - must be completed annually

• Safe Sport - the certificate should upload itself once the course is completed - I STRONGLY recommend that a PDF is saved as well in case the system hiccups - must be completed annually
• EPYSA Risk Management form - check all of the relevant boxes and sign w/ the mouse and submit - must be completed annually

Background Manager - if you are using this it is to help secure your PA child & criminal background checks - and houses them (you can upload if you have them separately) and the system will alert you 90 days prior to expiration of these documents at the 5-year mark.
